Simple steps to access the care you need

Support at Home Program

Staying safe, comfortable, and independent at home is important to you — and it’s important to us too. The Support at Home Program is here to make that possible.

It provides government funding for older Australians who need a little extra help with daily living, health needs, or specialist support.

If you’re finding some tasks harder than they used to be, or if you want reassurance that the right care will be there when you need it, here’s how to get started.

How to get started

You may be eligible if you:

  • Are 65 years or older (or 50+ for Aboriginal and Torres Strait Islander people).
  • Are living at home (not in residential aged care).
  • Need support to continue living safely and independently.

Eligibility is assessed by the government, not providers — but we can guide you through the process.

The first step is to register with My Aged Care, the national gateway for aged care services.

  • Call 1800 200 422, or

You’ll be asked some basic questions about your health, lifestyle, and support needs.

After applying, an assessor will contact you to arrange a home visit (or phone assessment in some cases).

They will:

  • Talk with you about what’s working well and where you need support.
  • Check your mobility, safety, and day-to-day challenges.
  • Recommend the types of services you may be eligible for under Support at Home.

After the assessment, you’ll receive a letter confirming:

  • The level of support you are eligible for.
  • The types of services you can access.
  • How much funding the government will contribute toward your care.

Once approved, you can select a provider like Harwell to deliver your services. We’ll:

  • Work with you to design a personalised care plan.
  • Explain your funding in plain language, so you know what’s covered.
  • Begin services when you’re ready.

From your very first visit, we’ll make sure you feel supported, safe, and respected. Services can grow or change with your needs — so your care always fits your lifestyle.
